Unstable fractures usually require open or closed reduction and fixation either directly/internally onto the fracture using Kirschner wires, cerclage wires, screws or miniplates or indirectly/externally by ligamentotaxis using external fixators which can be dynamic or Comminuted- Difficult fracture to treat, (Pilon fracture of base of middle phalanx). Treatment methods include, buddy strapping and early active motion, immobilization, extension block splinting, open reduction and internal fixation, external fixation, silicone prostheses, and fusion. Hyperextension injury involving the PIP of the finger can avulse the volar plate which is commonly associated with a volar avulsion fracture at the base of the middle phalanx. When the volar avulsion fracture involves a significant portion of the articular surface, instability and dorsal dislocation of middle phalanx can occur.

Methods: Thirteen patients with acute unstable PIP joint fracture-dislocation (n = 8), pilon fracture (n = 2), or comminuted shaft fractures of a middle phalanx with or without PIP joint extension (n = 3) were enrolled. The alternative fixator used a plastic syringe to maintain distraction force instead of the commonly used rubber bands. Results: There were 26 PIP fracture-dislocations (3 chronic, average 6 weeks) and 8 PIP pilon injuries. The average follow-up period was 16 months (range, 6-84 months).
